Intercooler top pipes often fail, so would be my first check. Dead easy to replace too, which is nice. 2012 SDV6 - it's missing a couple of cylinders

Top passenger side intercooler hose with a 6cm split.

New hose installed, car runs much smoother and responsive now (and less soot out the back as you'd expect), and no error codes appearing.
