holes in intercooler

hi guys, we have a problem with a 57 plate 2.7 that keeps blowing holes in the intercooler pipes (the plastic ones by the sensor on top of the engine) thick black smoke and no power, could this be a blocked particle filter or faulty egr valve? there are no fault lights on the dash, would really appreciate advice where to start looking for the cause,

cheers

Have the PCM flashed back to standard. Sounds as though there is likely a nasty remap on there
